Registration Information

Please complete the form, print a copy, and hit the submit button to complete the registration. You should immediately receive a confirmation confirming your registration. If you do not receive a confirmation, check your email address and resubmit. Submit a copy of the form to your financial officer and send a check for professional development to the NCADA ($85.00 per class if paid by 7/1/13). Registration is (100.00 per class) if post marked after July 1st. Instructional materials will ordered on July 3rd. Cancellation fee is $15.00 per class.
